<br><strong>Location:</strong> North Austin, TX (Candidates must reside in the North Austin area)<br />
<strong>Employment Type:</strong> Full-Time | Direct Hire<br />
<strong>Schedule:</strong> Monday â Friday | 8:00 AM â 5:00 PM<br />
<strong>Compensation:</strong> $90,000 â $110,000 annually (based on experience)<br />
<strong>Bonus:</strong> Up to $20,000, paid in installments<br> 

<br><strong>Position Overview</strong><br> 

<br><strong>Wheeler Staffing Partners </strong>is seeking an experienced Patient Care Manager (RN) to oversee clinical operations and lead a team of home health professionals serving patients throughout the North Austin area. This leadership role is responsible for ensuring high-quality patient care, supporting clinical staff, maintaining regulatory compliance, and driving positive patient outcomes.<br> 

<br>The ideal candidate will have prior home health experience, proven leadership capabilities, strong clinical knowledge, and a passion for delivering exceptional patient-centered care. This position serves as a key member of the leadership team and plays a critical role in managing day-to-day clinical operations, staff performance, care coordination, and quality initiatives.<br> 

<br><strong>Key Responsibilities</strong><br> 

<br><strong>Clinical Operations & Patient Care Management</strong><br> 

<br>&bull; Oversee the delivery of home health services to ensure quality patient care and regulatory compliance.<br />
&bull; Review and approve physician orders and follow up on denied or pending orders.<br />
&bull; Facilitate and manage case conferences, including follow-up activities and care coordination.<br />
&bull; Review Start of Care (SOC), Resumption of Care (ROC), Recertification, Evaluation, and Discharge documentation.<br />
&bull; Review and approve Plan of Care (POC) updates and modifications.<br />
&bull; Monitor patient episodes for changes in nursing, therapy, aide services, and utilization patterns.<br />
&bull; Review clinical documentation for accuracy, completeness, and compliance.<br />
&bull; Conduct utilization management reviews and monitor patient outcomes.<br />
&bull; Participate in recertification and discharge decision-making processes.<br />
&bull; Review, edit, and finalize clinical records and documentation.<br> 

<br><strong>Leadership & Team Management</strong><br> 

<br>&bull; Supervise and support a team of RN and therapist Case Managers.<br />
&bull; Provide coaching, mentoring, performance feedback, and professional development opportunities.<br />
&bull; Ensure staff compliance with agency policies, procedures, and clinical standards.<br />
&bull; Conduct staff education, competency evaluations, and ongoing training activities.<br />
&bull; Approve PTO requests and assist with staffing oversight.<br />
&bull; Support onboarding and development of new clinical team members.<br />
&bull; Foster a collaborative, patient-focused culture within the organization.<br> 

<br><strong>Quality Assurance & Compliance</strong><br> 

<br>&bull; Participate in Quality Assurance and Performance Improvement (QAPI) activities.<br />
&bull; Ensure compliance with federal, state, and local regulations governing home health services.<br />
&bull; Monitor documentation quality, clinical outcomes, and operational performance metrics.<br />
&bull; Identify opportunities for process improvement and implement corrective actions when necessary.<br />
&bull; Collaborate with interdisciplinary teams to support optimal patient care outcomes.<br> 

<br><strong>Administrative Responsibilities</strong><br> 

<br>&bull; Support scheduling, reporting, care coordination, and operational workflows.<br />
&bull; Maintain accurate records and documentation within electronic medical record systems.<br />
&bull; Communicate effectively with physicians, patients, families, and healthcare partners.<br />
&bull; Assist with workload management, staffing coordination, and resource allocation.<br> 

<br><strong>Required Qualifications</strong><br> 

<br><strong>Education</strong><br> 

<br>&bull; Associate Degree in Nursing (ADN) required.<br />
&bull; Bachelor's Degree in Nursing (BSN), Healthcare Administration, or a related field preferred.<br> 

<br><strong>Licensure</strong><br> 

<br>&bull; Active and unrestricted Registered Nurse (RN) license in the State of Texas required.<br> 

<br><strong>Experience</strong><br> 

<br>&bull; Minimum 1 year of Home Health experience required.<br />
&bull; Minimum 1 year of leadership experience required, including supervisory, management, team lead, or clinical lead responsibilities.<br />
&bull; Experience overseeing clinical staff, care planning, and patient care delivery preferred.<br>
