Wheeler Staffing Partners is hiring a Service Coordinator in Chaparral, NM. The Service Coordinator assists with the coordination, evaluation and planning of home care services. Home care may include assistance with the activities of daily living, housecleaning, laundry, meal preparation, transportation, companionship, and respite. Service Coordinators ensure that employees deliver services in a caring and respectful manner, in accordance with relevant Agency policies and industry standards. Apply today!
Service Coordinator Job Responsibilities:
  • Ensure Agency policies and procedures and industry standards and regulations are followed.
  • Assist with the coordination, evaluation and planning of home care services.
  • Assist the Administrator with administrative activities and optimal use of resources.
  • Develop, implement, and evaluate strategic plans, goals and objectives.
  • Coordinate staffing and scheduling activities.
  • Develop and instruct employees in the use of practices, procedures, and equipment.
  • Prepare budget reports based on compiled financial and statistical records.
  • Liaise with community resources and other agencies.
Service Coordinator Job Requirements:
  • Associates/Bachelors preferred; high school diploma/GED required
  • Must have a valid New Mexico driver's license
  • Must comply with HIPAA policies
  • Must be available as needs arise and possess a high level of flexibility 
  • Knowledge of standards of practice and scope of practice, in accordance with state regulations
  • Comprehensive knowledge of other health care disciplines and their role in client and family care.
  • Knowledge of principles and processes for providing client and personal care services, including needs determinants, meeting quality standards and evaluation of client satisfaction.
  • Knowledge of clerical procedures such as maintaining records and completing forms.
