Job Title: Registered Nurse (RN) – Field Nurse, Home Health
Location: Loveland, CO
Employment Type: Full-Time
Work Environment: Onsite, Home Health
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Productivity Expectation: 28 points per week
Coverage Area: West of I-25, CO
Compensation
Estimated Annual Compensation: $95,000 – $105,000
This position is paid on a pay?per?point basis. The compensation reflected is an estimated annual earning potential for full?time status.
Pay Per Visit Model
- Minimum expectation: 28 visits per week
- Typical range: 28–40 visits per week
Position Overview
Wheeler Staffing Partners is seeking a dedicated and compassionate Registered Nurse (RN) Field Nurse to support home health patients in the Loveland, CO area. This role focuses on delivering high?quality, patient?centered nursing care in the home setting while working collaboratively with an interdisciplinary healthcare team. The RN Field Nurse plays a key role in managing patient cases, coordinating care plans, and promoting positive clinical outcomes.
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and established care plans
- Conduct comprehensive patient assessments and develop individualized plans of care
- Coordinate and communicate patient care with physicians, therapists, and other healthcare professionals
- Monitor patient progress and adjust care plans as clinically appropriate
- Educate patients and families on disease processes, medications, and treatment plans
- Complete accurate, timely documentation in compliance with regulatory and clinical standards
- Promote patient independence, safety, and overall quality of life
Qualifications
- Graduate of an accredited school of professional nursing
- Minimum of one (1) year of Registered Nurse experience
- Home health nursing experience preferred, but not required
Required Certifications and Licensure
- Active Registered Nurse (RN) license in the State of Colorado
- Current CPR certification
- Valid driver’s license
- Reliable transportation and current auto insurance
- Ability to travel to patient homes within the assigned coverage area
