Coverage Area: Wyoming and Susquehanna Counties
Compensation
Estimated Annual Earnings: $85,000 – $100,000 per year
This role is compensated on a per-visit basis. The annual salary range listed is an estimated earning potential.
Paid Per Visit Model:
- Minimum productivity: 28 visits per week
- Typical range: 28–40 visits per week
Position Summary
Wheeler Staffing Partners is seeking a compassionate and experienced Registered Nurse (RN) Case Manager – Home Health to support patients in the Dickson City, PA area. This role is responsible for managing patient care in a home health setting, coordinating services, and ensuring high-quality, patient-centered outcomes.
The RN Case Manager works independently in the field while collaborating with an interdisciplinary care team to deliver skilled nursing services, promote patient independence, and support overall quality of life.
Essential Duties and Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Perform comprehensive patient assessments and develop individualized plans of care
- Manage assigned patient caseloads and coordinate care services
- Communicate and collaborate with physicians, therapists, and other healthcare professionals
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on medications, disease processes, and treatment plans
- Complete accurate and timely clinical documentation in compliance with state and federal regulations
- Promote patient safety, independence, and quality-of-life outcomes
Qualifications
- Graduate of an accredited school of professional nursing
- Active Registered Nurse (RN) license in the state of practice
- Minimum of one (1) year of RN experience
- Home health experience preferred, but not required
Certifications, Licensure, and Requirements
- Current RN license in the applicable state
- Current CPR certification
- Valid driver’s license with reliable transportation and current auto insurance
- Ability to travel to patient homes within the assigned coverage area
