Registered Nurse (RN) – Home Health
Location: San Marcos, CA
Schedule: Monday to Friday, 8:00 AM – 5:00 PM
Compensation: $95,000 – $120,000+ annually (DOE)
Sign-On Bonus: Substantial – details upon inquiry
About the Role
Wheeler Staffing Partners is seeking a compassionate and skilled Registered Nurse (RN) – Home Health for our client in San Marcos, CA. This role offers the opportunity to provide high-quality, patient-centered care in the comfort of patients’ homes, ensuring they receive the support and treatment needed to achieve optimal health outcomes. The position provides a balance of autonomy and teamwork, allowing RNs to make meaningful connections with patients while working alongside an interdisciplinary healthcare team.
Key Responsibilities Patient Assessments: Conduct comprehensive in-home evaluations to determine physical, emotional, and psychosocial needs. Care Plan Management: Develop and implement personalized care plans in collaboration with physicians and healthcare teams. Medication Administration: Educate patients on medication adherence, monitor for side effects, and administer prescribed treatments. Wound Care: Provide wound care, including dressing changes and monitoring progress. Chronic Disease Management: Assist patients with diabetes, hypertension, COPD, heart failure, and other chronic conditions through monitoring and intervention. Patient and Family Education: Teach patients and caregivers about disease management, treatment plans, and home safety. Care Coordination: Serve as a liaison between patients, families, physicians, and other care providers to ensure seamless transitions of care. IV Therapy: Administer intravenous medications and fluids, providing necessary patient education. Palliative & End-of-Life Care: Offer compassionate care focused on pain and symptom management, emotional support, and guidance for families. Regulatory Compliance: Maintain accurate, up-to-date documentation per agency policies, state regulations, and payer requirements. Supervision: Oversee and guide LPNs and Home Health Aides to ensure continuity and quality of care. Emergency Response: Identify and respond to medical emergencies with immediate interventions. Infection Control: Adhere to best practices in infection control, educating patients and families on prevention. Quality Improvement: Participate in quality assurance initiatives to improve patient outcomes.
Qualifications
Education: Graduate of an accredited school of professional nursing. Experience: Minimum 1 year of RN experience (home health experience preferred). Licensure: Current RN license in California. Valid driver’s license and reliable transportation for patient visits. Current CPR certification (preferred). Skills: Strong clinical judgment, exceptional communication skills, and the ability to work both independently and collaboratively. Our Investment in You
