Position Overview
Wheeler Staffing Partners is seeking an experienced RN Patient Care Manager, Home Health for a direct hire opportunity in McKinney, TX. This leadership role is responsible for overseeing the daily operations of a home health team while ensuring the delivery of high-quality, patient-centered care. The RN Patient Care Manager will provide clinical leadership, supervise field staff, maintain regulatory compliance, and partner with patients, families, and healthcare providers to ensure exceptional outcomes.
The ideal candidate is a Texas-licensed Registered Nurse with a strong background in home health, proven leadership experience, and a passion for developing high-performing clinical teams.
Key Responsibilities
Clinical Leadership

Supervise and coordinate the delivery of home health services to ensure exceptional patient care and positive clinical outcomes.

Lead, mentor, coach, and support a multidisciplinary home health team while fostering a culture of accountability and continuous improvement.

Develop, review, and oversee individualized patient care plans based on physician orders and patient needs.

Monitor patient progress and adjust care plans as appropriate to support optimal clinical outcomes.

Serve as a clinical resource for field clinicians while promoting evidence-based practices and quality standards.
Operations & Compliance

Ensure compliance with all federal, state, and local regulations, Medicare Conditions of Participation, accreditation standards, and company policies.

Monitor quality metrics, patient outcomes, and performance indicators to support continuous improvement initiatives.

Conduct staff meetings, coaching sessions, and ongoing clinical education to promote professional development.

Collaborate with physicians, referral sources, patients, caregivers, and interdisciplinary teams to ensure coordinated, patient-centered care.

Support recruiting, onboarding, performance management, employee engagement, and staff development initiatives.
Administrative Responsibilities

Oversee scheduling, staffing, productivity, documentation, and daily operational workflows.

Assist with billing, reporting, and operational performance initiatives.

Maintain accurate clinical and operational documentation.

Identify opportunities to improve efficiency, regulatory compliance, quality, and patient satisfaction.
Required Qualifications
-
Active Registered Nurse (RN) license in the State of Texas.

Minimum of two (2) years of Home Health experience.

Minimum of two (2) years of Home Health supervisory or management experience.

Strong knowledge of home health regulations, clinical operations, and patient care standards.

Proven leadership experience managing and developing clinical teams.

Excellent organizational, critical thinking, and decision-making skills.

Strong verbal and written communication skills.

Ability to manage multiple priorities in a fast-paced healthcare environment.

Proficiency with Microsoft Office and electronic medical record (EMR) systems.
Preferred Qualifications
-
Bachelor's degree in Nursing, Healthcare Administration, or a related field.

Experience leading quality improvement initiatives within a home health organization.

Knowledge of Medicare home health reimbursement and regulatory requirements.

Experience with home health EMR platforms.
