Patient Care Manager (RN) – Home Health
Location: North Austin, TX (Candidates must reside in the North Austin area)
Employment Type: Full-Time | Direct Hire
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Compensation: $90,000 – $110,000 annually (based on experience)
Bonus: Up to $20,000, paid in installments
Position Overview
Wheeler Staffing Partners is seeking an experienced Patient Care Manager (RN) to oversee clinical operations and lead a team of home health professionals serving patients throughout the North Austin area. This leadership role is responsible for ensuring high-quality patient care, supporting clinical staff, maintaining regulatory compliance, and driving positive patient outcomes.
The ideal candidate will have prior home health experience, proven leadership capabilities, strong clinical knowledge, and a passion for delivering exceptional patient-centered care. This position serves as a key member of the leadership team and plays a critical role in managing day-to-day clinical operations, staff performance, care coordination, and quality initiatives.
Key Responsibilities
Clinical Operations & Patient Care Management
• Oversee the delivery of home health services to ensure quality patient care and regulatory compliance.
• Review and approve physician orders and follow up on denied or pending orders.
• Facilitate and manage case conferences, including follow-up activities and care coordination.
• Review Start of Care (SOC), Resumption of Care (ROC), Recertification, Evaluation, and Discharge documentation.
• Review and approve Plan of Care (POC) updates and modifications.
• Monitor patient episodes for changes in nursing, therapy, aide services, and utilization patterns.
• Review clinical documentation for accuracy, completeness, and compliance.
• Conduct utilization management reviews and monitor patient outcomes.
• Participate in recertification and discharge decision-making processes.
• Review, edit, and finalize clinical records and documentation.
Leadership & Team Management
• Supervise and support a team of RN and therapist Case Managers.
• Provide coaching, mentoring, performance feedback, and professional development opportunities.
• Ensure staff compliance with agency policies, procedures, and clinical standards.
• Conduct staff education, competency evaluations, and ongoing training activities.
• Approve PTO requests and assist with staffing oversight.
• Support onboarding and development of new clinical team members.
• Foster a collaborative, patient-focused culture within the organization.
Quality Assurance & Compliance
• Participate in Quality Assurance and Performance Improvement (QAPI) activities.
• Ensure compliance with federal, state, and local regulations governing home health services.
• Monitor documentation quality, clinical outcomes, and operational performance metrics.
• Identify opportunities for process improvement and implement corrective actions when necessary.
• Collaborate with interdisciplinary teams to support optimal patient care outcomes.
Administrative Responsibilities
• Support scheduling, reporting, care coordination, and operational workflows.
• Maintain accurate records and documentation within electronic medical record systems.
• Communicate effectively with physicians, patients, families, and healthcare partners.
• Assist with workload management, staffing coordination, and resource allocation.
Required Qualifications
Education
• Associate Degree in Nursing (ADN) required.
• Bachelor's Degree in Nursing (BSN), Healthcare Administration, or a related field preferred.
Licensure
• Active and unrestricted Registered Nurse (RN) license in the State of Texas required.
Experience
• Minimum 1 year of Home Health experience required.
• Minimum 1 year of leadership experience required, including supervisory, management, team lead, or clinical lead responsibilities.
• Experience overseeing clinical staff, care planning, and patient care delivery preferred.
