Home Health - Physical Therapist
Location(s): Desoto, Tx areaÂ
Days Monday – Friday 8a-5p with flexibilityÂ
Pay Rate: $95k - 110k/yr DOE
Milage reimbursementÂ
We are actively seeking a dedicated and skilled Physical Therapist to join our team. As a Home Health - Physical Therapist, you will play a crucial role in helping patients recover from injuries and illnesses, improve their mobility, and regain independence in their daily lives. Â This role will work under the supervision of licensed physical therapists to provide therapeutic exercises and modalities to patients.

Qualifications:
•   Be able to implement care plans and communicate any findings or changes regarding patients back to the Physical Therapist and physician.
•   Ability to educate patients and their families in basic home safety and medical equipment safety.
•   Strong understand of DME and ability to educate and implement
•   Strong understand of physical rehabilitation and exercise scienceÂ
•   Ability to successfully guild patients through Therapeutic movements/ exercisesÂ
•   Experience working with Geriatric populationÂ
•   Home health experience a plusÂ
Required Certifications and Licensures:
•   Associate degree from an accredited school of Physical Therapy
•   Have a valid license, registration, or permit to practice in state of agency operation. (TX)
•   Must possess and maintain valid CPR certification while employed in a clinical role.
•   Must be a licensed driver who can travel to all business locations.
