Home Health Physical Therapist
We are looking for a compassionate and skilled Physical Therapist (LVN) to join our Home Health Team, dedicated to providing high-quality, patient-centered care in the comfort of patients' homes. The PT will work closely with an interdisciplinary healthcare team, contributing to patient wellness, chronic condition management, and enhanced health outcomes
Physical Therapist - PT (FT)
Location: Chisago City, MN
Area Coverage: (North Branch, Andover, Princeton, East Bethel, Cambridge)Â
Shift: Mon- Fri 8am-5pm
Pay Range: $115k – 125k+ /yr   (scaled DOE)
Significant sign on bonus available
On call requirement – Yes
Key Responsibilities
Qualifications
Excel as a Home Health Physical TherapistLicenses and Certifications:
Current state license to practice as a physical therapist, meeting professional standards.
Valid CPR certification throughout employment.
Valid driver’s license with reliable transportation for travel to patient locations.

Key Responsibilities
- Provide patient-centered physical therapy services in the home setting, addressing mobility, strength, and rehabilitation needs.
- Develop individualized care plans, implementing effective therapeutic interventions based on patient assessments.
- Collaborate with an interdisciplinary team to achieve optimal patient outcomes.
- Educate patients and their families on therapy techniques and strategies to promote recovery and independence.
- Maintain accurate, thorough documentation of patient visits and progress.
Qualifications
Excel as a Home Health Physical Therapist
- Minimum of 9 months of experience as a physical therapist (community or home health experience preferred).
- Educational Requirements:
- Master’s degree in physical therapy from a CAPTE-accredited program.
