Location: Loveland, CO
Coverage Area: Greeley, Evans, Ault, Severance, Lochbuie, Hudson, Windsor, and surrounding communities
Employment Type: Full-Time | Direct Hire
Work Arrangement: Field-Based | Home Health
Schedule: Monday–Friday | 8:00 AM–5:00 PM
Compensation: $100,000–$112,000 annually (DOE)
Sign-On Bonus: Available
On-Call Requirement: Yes
Position Overview
Wheeler Staffing Partners is seeking a compassionate Physical Therapist, Home Health to provide skilled, patient-centered care throughout the Loveland, CO area. This field-based role is responsible for evaluating patients, developing individualized treatment plans, improving mobility and function, and collaborating with an interdisciplinary team to deliver exceptional home health services.

Position Overview
Wheeler Staffing Partners is seeking a compassionate Physical Therapist, Home Health to provide skilled, patient-centered care throughout the Loveland, CO area. This field-based role is responsible for evaluating patients, developing individualized treatment plans, improving mobility and function, and collaborating with an interdisciplinary team to deliver exceptional home health services.
Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide skilled therapy services to improve mobility, strength, balance, and functional independence.
- Educate patients and caregivers on exercises, mobility, safety, and recovery strategies.
- Collaborate with physicians and interdisciplinary care teams to coordinate patient care.
- Monitor patient progress and adjust treatment plans as needed.
- Complete accurate and timely clinical documentation.
- Maintain compliance with company policies and regulatory standards.
Required Qualifications
- Master's degree in Physical Therapy from a CAPTE-accredited program.
- Current Colorado Physical Therapist license.
- Minimum of nine (9) months of Physical Therapist experience (home health preferred).
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Strong clinical, communication, and documentation skills.
- Home health or community-based rehabilitation experience.
- Experience with Medicare home health documentation.
- Experience working with geriatric or medically complex patients.
- Ability to travel throughout the assigned coverage area.
- Ability to stand, walk, bend, lift, and assist patients with mobility.
- Ability to safely transport therapy equipment and supplies.
