Job Title: Physical Therapist – Home Health
Employment Type: Full-Time
Location: Denver, CO
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Compensation: $100,000 – $115,000 annually (estimated, based on per-point model)
Sign-On Bonus: Available

Position Overview
Wheeler Staffing Partners is seeking a compassionate and skilled Home Health Physical Therapist to provide high-quality, patient-centered care in the home setting. This role focuses on improving mobility, strength, and overall functional outcomes while supporting patients in achieving independence and enhanced quality of life.

The ideal candidate is collaborative, detail-oriented, and passionate about delivering exceptional care in a field-based environment.

Schedule & Compensation Model
  • Full-Time schedule
  • Sign-on bonus available
  • On-call requirement: Yes

Key Responsibilities
  • Provide patient-centered physical therapy services in the home setting
  • Assess patient mobility, strength, and functional limitations to develop individualized care plans
  • Implement therapeutic interventions to support rehabilitation and recovery
  • Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
  • Educate patients and families on treatment plans, exercises, and strategies for independence
  • Maintain accurate, timely documentation of patient visits and progress
  • Monitor patient response to treatment and adjust care plans as needed

Qualifications
  • Minimum 9 months of experience as a Physical Therapist (home health or community-based experience preferred)
  • Strong clinical, organizational, and communication skills
  • Ability to work independently in a field-based setting

Education
  • Master’s degree in Physical Therapy from a CAPTE-accredited program (required)

Certifications & Requirements
  • Active Physical Therapist license in Minnesota (required)
  • Active Wisconsin PT license (required) OR willingness to obtain
  • Current CPR certification
  • Valid driver’s license with reliable, insured transportation
  • Ability to travel to patient homes within assigned territory
  • Must pass background check and drug screen
