Position Overview
Wheeler Staffing Partners is seeking experienced Home Health Physical Therapists (PTs) for full-time roles serving the Fort Worth, Irving, DeSoto, Mesquite, and South Dallas regions. The PT will provide skilled in-home physical therapy services aimed at improving mobility, independence, safety, and overall functional outcomes. This role includes patient assessment, development of individualized treatment plans, and ongoing clinical interventions delivered in the patient’s home environment.

Position Overview
Wheeler Staffing Partners is seeking experienced Home Health Physical Therapists (PTs) for full-time roles serving the Fort Worth, Irving, DeSoto, Mesquite, and South Dallas regions. The PT will provide skilled in-home physical therapy services aimed at improving mobility, independence, safety, and overall functional outcomes. This role includes patient assessment, development of individualized treatment plans, and ongoing clinical interventions delivered in the patient’s home environment.
Key Responsibilities

Perform comprehensive patient assessments and develop individualized therapy plans.

Deliver therapeutic interventions including strengthening exercises, gait training, balance work, and mobility training.

Educate patients and caregivers on home exercise programs, safety strategies, equipment use, and fall prevention.

Maintain timely and accurate clinical documentation for all patient encounters.

Communicate regularly with the care team to support coordinated treatment and patient progress.

Travel within assigned service areas to complete home visits.
Qualifications

Master’s or Doctorate degree in Physical Therapy (CAPTE-accredited).

Active Physical Therapist (PT) license in the state of Texas.

Current CPR certification (must remain active).

Valid driver’s license and reliable transportation.

Strong understanding of physical rehabilitation and exercise science.

Experience with geriatric patients preferred; home health experience strongly preferred.
