Licensed Vocational Nurse (LVN) - Home Health Team
Position Overview
We are looking for a compassionate and skilled Licensed Vocational Nurse (LVN) to join our Home Health Team, dedicated to providing high-quality, patient-centered care in the comfort of patients' homes. The LVN will work closely with an interdisciplinary healthcare team, contributing to patient wellness, chronic condition management, and enhanced health outcomes.
Home health LVN/LPV (FT)
Location: St. Paul, MN
Schedule: Mon – Fri 8a – 5pm with flexibility
Pay range: $30 - $40/hr (scaled DOE)
Differential available – no
On call requirement – Yes
Key Responsibilities
- Deliver patient-centered care in the home setting, conducting nursing visits as prescribed by physicians.
- Monitor and manage patients’ health conditions, supporting recovery and improving quality of life.
- Maintain accurate and comprehensive patient records, ensuring detailed documentation of visits, care provided, and patient responses.
- Collaborate with other healthcare professionals, including RNs, physicians, and specialists, to optimize patient care plans and achieve care goals.
- Educate patients and families on managing chronic conditions, medications, and lifestyle choices to support health outcomes.
Qualifications
- Licensure:
- Current Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the state of practice.
- Experience:
- 1 year of clinical experience and 1 year of home health experience.
- Education:
- Graduate of an accredited practical or vocational nursing program.
- Certifications:
- CPR/BLS certification (required, must be maintained throughout employment).
- Valid driver’s license with the ability to travel to various patient locations as needed.
