Job Title: Licensed Vocational Nurse (LVN) – Home Health
Location: Dallas, TX
Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Job Summary
Wheeler Staffing Partners is seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join a Home Health team. This role is responsible for delivering high-quality, patient-centered care in patients’ homes. The LVN will collaborate closely with an interdisciplinary healthcare team to promote wellness, manage chronic conditions, and support optimal health outcomes.
Key Responsibilities
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Conduct routine nursing visits and assessments
- Monitor patient conditions and report changes to the care team
- Administer medications and treatments as prescribed
- Educate patients and caregivers on disease management and care plans
- Maintain accurate, timely, and compliant medical documentation
- Collaborate with physicians, therapists, and other healthcare professionals
- Ensure adherence to all clinical and regulatory standards
Qualifications
- Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of Texas
- Minimum of one (1) year of clinical experience
- Minimum of one (1) year of home health experience
- Ability to perform nursing visits and maintain accurate medical records
- Strong communication, organizational, and patient care skills
Required Certifications & Licenses
- Graduate of an accredited practical or vocational nursing program
- Current CPR certification (must be maintained throughout employment)
- Valid driver’s license and reliable transportation with the ability to travel to patient locations
