Hospice Registered Nurse (RN) 
 
  • Role: Hospice RN Case Manager
  • Schedule: Monday – Friday, 8:00 AM to 5:00 PM
  • On Call Requirements .
  • Pay Range: $75k - 90k/yr with flexibility for highly experienced candidates.
  • Mileage Reimbursement: $0.62/mile

Service Area:
  • Cherokee and Copy Counties: Primary service area.


About the Role
We are seeking a compassionate, dedicated, and highly skilled Registered Nurse (RN) to join our Home Health team. As an RN in our Home Health/ Hospice program, you will provide high-quality, patient-centered care in the comfort of patients' homes. This role involves delivering skilled nursing services, coordinating care plans, and empowering patients and their families to achieve optimal health outcomes. You'll work collaboratively with an interdisciplinary healthcare team while enjoying the autonomy and meaningful connections unique to home health care.


Key Responsibilities
Comprehensive Patient Assessments: Perform detailed in-home assessments to evaluate patients’ physical, emotional, and psychosocial needs.
Care Plan Development and Implementation: Create individualized care plans tailored to each patient’s specific needs, conditions, and goals, collaborating with physicians and other care providers as necessary.
Medication Management: Administer medications, provide education on medication adherence, and monitor for potential side effects or interactions.
Wound Care and Treatment: Provide wound care management, including cleaning, dressing changes, and monitoring progress.
Chronic Disease Management: Support patients with chronic conditions such as diabetes, hypertension, COPD, and heart failure through education, monitoring, and intervention.
Patient and Family Education: Teach patients and families about disease processes, treatment plans, home safety, and self-care to promote independence.
Coordination of Services: Act as the liaison between patients, families, physicians, and other healthcare providers, ensuring seamless transitions of care.
IV Therapy: Administer intravenous medications and fluids and provide patient education about IV therapy as needed.
Palliative and End-of-Life Care: Deliver compassionate care for patients and families, addressing pain and symptom management, emotional support, and resources for coping.
Documentation and Compliance: Maintain accurate and timely documentation in compliance with agency policies, state regulations, and payer requirements.
Supervision of LPNs and Home Health Aides: Provide oversight and guidance to team members to ensure continuity and quality of care.
Emergency Response: Identify and respond to medical emergencies, providing immediate intervention and notifying appropriate care providers.
Infection Control: Adhere to infection control guidelines and educate patients and families on preventing the spread of illness.
Continuous Quality Improvement: Participate in quality assurance activities to improve patient outcomes and enhance service delivery.

Experience: Minimum of 1-2 years of recent hospice experience is required due to the high acuity level. Familiarity with admissions and cross-training is essential as this is a small team environment that promotes collaboration and teamwork. 

Qualifications
Education: Graduate of an accredited school of professional nursing.
Experience: Minimum of one year as an RN (Hospice experience preferred).

Licensure:
Current RN license to practice in GA.
Valid driver’s license and reliable transportation to travel between patients’ homes.
Current CPR certification (preferred).
Skills: Strong clinical judgment, exceptional communication skills, and the ability to work independently and collaboratively.
