Job Title: Physical Therapist – Home Health
Location: Methuen, MA
Coverage Area: Nashua, NH to Bedford, NH and surrounding areas
Employment Type: Full-Time
Work Environment: Onsite, Home Health
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (On-call required)
Compensation Estimated Annual Salary: $96,000 – $117,000
Bonus: $20,000 Sign-On Bonus
Pay Model: Per-Point Compensation Structure (estimated annual earnings based on full-time productivity)
Position Overview Wheeler Staffing Partners is seeking a compassionate and skilled Physical Therapist to join a Home Health team serving patients across Massachusetts and New Hampshire. This role focuses on delivering high-quality, patient-centered care in the home setting, supporting rehabilitation, chronic condition management, and improved health outcomes.
The Physical Therapist will collaborate with an interdisciplinary healthcare team to develop and implement individualized care plans while promoting patient independence and overall wellness.

The Physical Therapist will collaborate with an interdisciplinary healthcare team to develop and implement individualized care plans while promoting patient independence and overall wellness.
Key Responsibilities
- Provide skilled physical therapy services in patients’ homes, focusing on mobility, strength, and functional improvement
- Develop and implement individualized treatment plans based on patient assessments
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
- Educate patients and caregivers on therapy techniques, safety, and recovery strategies
- Maintain accurate and timely documentation of patient visits, progress, and outcomes
- Manage a designated coverage area and travel between patient locations
Qualifications
- Minimum of 9 months of experience as a Physical Therapist (home health or community-based experience preferred)
- Strong clinical assessment and care planning skills
- Ability to work independently in a home-based setting
- Excellent communication and patient engagement skills
Education
- Master’s degree in Physical Therapy from a CAPTE-accredited program
Licensure & Certifications
- Active Physical Therapist license (or ability to obtain) in Massachusetts and New Hampshire (dual licensure required)
- Current CPR certification
- Valid driver’s license and reliable transportation
