Position Summary
Wheeler Staffing Partners is seeking a Home Health Physical Therapist to join a compassionate, patient-centered healthcare team in Brooklyn Park, MN. Key Responsibilities

Deliver physical therapy services in a home health setting, addressing patients’ rehabilitation and mobility needs.

Conduct thorough assessments and develop individualized, goal-oriented treatment plans.

Collaborate with an interdisciplinary healthcare team to ensure effective, coordinated care.

Educate patients and families on therapeutic exercises, recovery techniques, and safety practices.

Maintain accurate documentation of all patient visits, progress notes, and treatment outcomes.

Promote patient independence and improved quality of life through consistent, compassionate care.
Required Qualifications

Education: Master’s degree in Physical Therapy from a CAPTE-accredited program.

Experience: Minimum of 9 months of experience as a licensed Physical Therapist (home health or community-based experience preferred).

Licenses/Certifications:

Active Physical Therapy license in the state of Minnesota.

Current CPR certification (must be maintained during employment).

Valid driver’s license and reliable transportation for travel to patient homes.

Prior experience in home health or community-based care settings.

Strong interpersonal, communication, and time management skills.

Ability to work independently while collaborating effectively within a healthcare team.
