Job Title: Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) – Home Health
Location: Loveland, CO
Employment Type: Full-Time
Work Environment: Onsite, Home Health
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
On-Call Requirement: Yes
Mileage Reimbursement: Yes
Coverage Area: East of I-25, south of Highway 66 (Southeast quadrant)
Compensation
Estimated Annual Compensation: $65,000 – $75,000
The compensation reflected is an estimate of annual earnings for full-time status. This role is compensated on a per-visit basis, providing opportunity for increased earnings based on visit volume.
Position Overview
Wheeler Staffing Partners is seeking a compassionate and dependable Licensed Practical Nurse or Licensed Vocational Nurse (LPN/LVN) to join a home health team serving patients in the Loveland, CO area. This position is ideal for nursing professionals who are passionate about delivering individualized, high-quality care in the home setting. The LPN/LVN will work independently in the field while collaborating with an interdisciplinary healthcare team to support positive patient outcomes.
Key Responsibilities
- Conduct skilled nursing visits in patients’ homes in accordance with physician orders and individualized care plans
- Assess, monitor, and evaluate patient conditions, including chronic disease management and post?acute recovery
- Provide patient and family education related to medications, treatment plans, and lifestyle modifications
- Accurately document assessments, interventions, and patient progress in the electronic medical record
- Communicate effectively with physicians, case managers, and other members of the healthcare team
- Travel to patient homes within the assigned coverage area while maintaining efficient time management
- Participate in on?call rotation as required
Qualifications
- Graduate of an accredited practical or vocational nursing program
- Minimum of one (1) year of clinical nursing experience
- Home health experience preferred, but not required
Licensure and Certifications
- Active LPN or LVN license in the State of Colorado
- Current CPR or BLS certification
- Valid driver’s license with reliable transportation and current auto insurance
