Schedule: Saturday & Sunday (12-hour shifts) + Monday or Friday (8-hour shift)

Salary: $120,000 – $146,000 annually

Job Summary
Wheeler Staffing Partners is seeking an experienced and compassionate Registered Nurse (RN) for a weekend Home Health role. This position is responsible for delivering high-quality, patient-centered care in a home setting across a designated coverage area. The ideal candidate has strong home health experience and is proficient in OASIS EHR/EMR documentation.

Key Responsibilities
  • Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
  • Perform comprehensive patient assessments and ongoing evaluations
  • Accurately complete OASIS documentation and maintain compliant electronic medical records
  • Administer medications and treatments as prescribed
  • Educate patients and caregivers on disease management, medications, and care plans
  • Monitor patient progress and communicate changes to the interdisciplinary care team
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Ensure compliance with all regulatory, clinical, and safety standards

Coverage Area
Hugo, Merlin, Rand, Sunny Valley, Gold Hill, Rogue River, Wimer, Cave Junction, Dryden, Kerby, O'Brien, Redwood, Williams, Wonder, and Grants Pass

Qualifications
  • Graduate of an accredited school of professional nursing
  • Current Registered Nurse (RN) license in the state of Oregon
  • Minimum of one (1) year of RN experience
  • Required home health experience
  • Experience with OASIS EHR/EMR systems required
  • Strong clinical assessment, documentation, and communication skills

Required Certifications & Licenses
  • Active RN license in Oregon
  • Current CPR certification (must be maintained throughout employment)
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to travel throughout assigned coverage area
