Job Title: LVN / LPN – Home Health
Location: Culver City, CA
Coverage Area: San Fernando Valley & Los Angeles (float required)
Employment Type: Full-Time (32 hours/week)
Compensation: Pay Per Visit Model
Estimated Annual Earnings: $83,356 – $92,523+ (based on visit volume)
Position Overview
Wheeler Staffing Partners is seeking a compassionate and reliable Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in a home health setting. This role offers a flexible, per-visit compensation structure with the ability to increase earnings based on productivity.
Compensation & Schedule
- Per visit pay model with variable rates based on visit type
- Minimum productivity: 32 visits per week
- Example: 32 visits/week at $36 per visit = approximately $59,904 annually (base example)
- Opportunity to increase earnings with additional visits
- Schedule: Monday – Friday, with flexibility
- On-call rotation required
- Mileage reimbursement provided
Key Responsibilities
- Provide skilled nursing care in patients’ homes in accordance with physician orders and care plans
- Monitor patient conditions and support chronic disease management and recovery
- Document all visits, assessments, and interventions accurately in EMR systems
- Collaborate with interdisciplinary teams to coordinate patient care
- Educate patients and families on medications, treatment plans, and health management
- Travel within assigned territory to complete scheduled visits
Qualifications
- Active LVN or LPN license in the state of California
- Graduate of an accredited vocational or practical nursing program
- Minimum of 1 year of clinical nursing experience
- Home health experience preferred
- Current BLS/CPR certification
- Valid driver’s license and reliable transportation
