Location: North Houston, TX (covering Conroe, Spring, and Katy regions)
Employment Type: Full-Time | Contract-to-Hire
Compensation: $50,000 – $52,000 base salary + commission and bonuses
Schedule: Monday – Friday | Daytime hours | Local site travel required
Overview
Wheeler Staffing Partners is hiring a Community Outreach Representative on behalf of a leading home care organization in the North Houston area. This is an on-the-road position responsible for visiting hospitals, rehabilitation centers, and senior living communities to build relationships and generate client referrals for non-skilled home care services.
This role is ideal for a goal-driven professional with experience in sales, healthcare marketing, or business development who enjoys being out in the field and engaging with healthcare professionals. No patient care is required.

Base Salary: $50,000 – $52,000 annually

Commission: $50–$200 per admission

Monthly Bonus Opportunity: Earn an additional $1,000 for achieving 10+ admissions per month

Gas Stipend: $200 per pay period

Key Responsibilities
-
Develop and execute marketing strategies to increase awareness of home care services.

Build and maintain relationships with referral sources, including hospitals, skilled nursing facilities, and healthcare providers.

Conduct regular site visits to healthcare facilities to meet with case managers, directors of nursing, and discharge planners.

Represent the company at community events and networking functions.

Manage incoming referrals and admissions, ensuring a smooth and professional client experience.

Collaborate with internal teams to coordinate services and ensure client satisfaction.

Track sales activities, leads, and results in CRM tools and submit regular progress reports.

Achieve and exceed performance metrics, including 10–15 admissions per month.
Qualifications

Minimum 1 year of sales or business development experience (healthcare or home care industry preferred).

Strong relationship-building skills with healthcare professionals and community partners.

Proven ability to generate referrals and increase admissions.

Excellent communication, presentation, and organizational abilities.

Comfortable working independently and managing time efficiently while on the road.

Must have reliable transportation and a valid driver’s license for daily local travel in the Houston area.

Bachelor’s degree preferred but not required.
