Location: Chisago City, TX
Service Area: North Branch, Cambridge, Princeton, Andover
Pay: $105,000 - $115,000 per year
Schedule: Monday - Friday, 8 AM - 5 PM

Job Overview
We are seeking a dedicated Occupational Therapist (OT) to join our home health team. In this role, you will provide compassionate, individualized care to patients in their homes, helping them regain independence in daily activities, enhance motor function, and improve their overall quality of life. You will work closely with an interdisciplinary team to create and implement personalized treatment plans that promote long-term wellness.

Job Overview
We are seeking a dedicated Occupational Therapist (OT) to join our home health team. In this role, you will provide compassionate, individualized care to patients in their homes, helping them regain independence in daily activities, enhance motor function, and improve their overall quality of life. You will work closely with an interdisciplinary team to create and implement personalized treatment plans that promote long-term wellness.
Key Responsibilities
- Evaluate patients’ physical and functional abilities to develop personalized treatment plans.
- Provide hands-on therapy and recommend adaptive techniques to improve daily living skills.
- Educate patients and caregivers on home exercise programs, safety measures, and proper use of adaptive equipment.
- Monitor patient progress and adjust treatment plans as needed.
- Maintain clear and accurate documentation of patient visits and progress.
- Collaborate with other healthcare professionals to ensure comprehensive care.
- Travel to patient homes within the assigned service area to provide high-quality care.
- Bachelor's or Master's degree in Occupational Therapy from an accredited program.
- Active Occupational Therapist license in the state of Texas.
- Current CPR certification.
- Valid driver’s license with reliable transportation.
- Home health experience preferred but not required.
